Hoveton & Wroxham Medical Centre Annual Patient Survey
logo02

The new Patient Participation policy was available from April 2011, and requires practices to establish patient satisfaction surveys. As part of this work we are asking our patients for their views on our performance.

As such you have been selected to participate and we would be grateful if you could take time to complete this questionnaire. Your response will be in total confidence and we would like you to be open and honest in your response.

It will take around 5 minutes.
